Rooted Coaching, LLC ("Rooted") provides health and wellness coaching services and offers clients access to laboratory testing through Fullscript, a third-party platform. Lab tests ordered through Fullscript are authorized by an independent licensed physician and processed by an independent testing laboratory. Rooted is not a medical practice, and our coaches do not diagnose, treat, or prescribe for any medical condition.

Any review or discussion of lab results with a Rooted coach is for educational and wellness purposes only. It is not medical advice and does not replace evaluation by a licensed healthcare provider.

Rooted does not assume liability for abnormal, critical, or potentially life-threatening laboratory findings. You are responsible for reviewing your results and following up with your own licensed healthcare provider.

By ordering lab testing through Rooted or Fullscript, you acknowledge and agree that:

• Laboratory testing is provided for informational and educational purposes only.
• Rooted does not provide medical diagnosis or treatment through lab ordering, lab review, or coaching services.
• You will seek the advice of a qualified healthcare provider regarding any medical concerns or abnormal findings, and you will not delay or forgo medical care based on information from Rooted.
• Fullscript, the authorizing physician, and the testing laboratory are independent third parties and are not employees or agents of Rooted.
• Rooted is not liable for the acts, omissions, errors, or negligence of Fullscript, the authorizing physician, the testing laboratory, or their employees, agents, or contractors.
• You are responsible for reviewing your results promptly once they are available.



Critical Results
If a result comes back at a critical value, notification is handled by the authorizing physician and/or testing laboratory according to Fullscript's procedures. If a Rooted coach becomes aware of a critical or significantly abnormal result, we will advise you to seek prompt medical attention.
